A. Safe and Effective Care Environment
i. No open flames or combustibles in rooms utilizing oxygen
ii. Provision of suctioning and tracheostomy care using proper technique
iii. Endotracheal tube care- mouth care and inspection
iv. Anticoagulation bleeding precaution initiation
v. Ventilator settings and alarm verification & documentation
vi. Emergency medical equipment availability for ventilated patient
vii. Evaluation need of chemical or physical restraint with ventilated patient
B. Health Promotion and Maintenance
i. Teach persons about effects of smoking on pulmonary function
ii. Encourage those over 50 or with respiratory problems to receive influenza vaccination yearly.
iii. Detailed hourly respiratory assessment of ARDS patient
iv. Teach injury avoidance with anticoagulant therapy and VTE prevention methods
C. Psychosocial considerations
i. Assess effect of respiratory problem on patient’s ADLs, work, and leisure life
ii. Explain all procedures, restrictions, and follow up requirements to patients having tests
iii. Ventilated patient- pain management; reassurance that speech loss is temporary
iv. Family of ventilated patient- encourage communication with patient; patient can hear while chemically restrained
v. If intubation is a concern, discuss/support patient and family
D. Physiologic Integrity
i. Immediately report alterations in respiratory status; assess breathing effectiveness in those with SOB or altered mental status
ii. Application of oxygen to any hypoxic patient
iii. Pre & post procedural and operative management
iv. Ventilated patient- monitor nutrition; humidify oxygen; bag-valve mask ventilate if patient is distressed; check machine settings to prescribed therapy each shift
v. Use laboratory data and clinical manifestations to evaluate the adequacy of oxygenation and ventilatory interventions- prioritize
